PROCEDURE
FOR SUBMISSION OF LESSON FEEDBACK FORMS
The
Lesson Feedback Forms provide both a convenient and valuable record
of classroom activities led
by GK-12 Fellows under the guidance of the classroom teacher.
The form is available in Word and PDF format on the UTMSI GK-12
website (see Forms, Reports, Workshops).
The
purpose of the form is to:
1. enourage more communication betweeen the fellow and teacher
on activities they do together
2.
encourage reflection on the day's activities, and
3.
help keep a current and accurate account of the GK-12 activities
in the classroom.
In
order to save time, we suggest that the top part of the form be
filled out prior to the lesson and the last section of the form
be filled out by the master teacher during the lesson. Once the
lesson has been completed, the fellow and teacher will review the
bottom of the form together where additions and revisions can be
made. Only one form needs to be completed for each unique lesson
prepared by the Fellow. If the same lesson is presented several
times during the course of a single day, the form is completed only
once.
Forms
should be promptly submitted to Ken Dunton by the Fellows.
